We have revised the burial registration forms to serve you better and to be able to provide a email receipt for your records.

When you first click on the burial registration graphic, instead of being taken directly to the form itself, you will be prompted to select the type of burial you are wishing to report.  This change will allow us to make quicker updates to the form going forward should reporting requirements from the State of Illinois change.
